Insurance Information
Many clients ask, “Will my insurance cover my visit?”
There is a process to find out.
First Step:
It’s your responsibility to call your insurance provider and ask:
- “Is Clayton Lessor, LPC an approved provider under my plan?”
- “What is my deductible?”
- “Do I need authorization?”
(Failure to complete this First Step may result in full payment out-of-pocket and/or a rescheduled visit.)

Get the paperwork “out-of-the-way!” Please fill out the attached new client information form!
Please note: Appointments must be verbally confirmed prior to visit. We do not have the capacity to provide childcare; children are not allowed in counseling sessions unless they are the client of the session. Children may not be left unattended in the waiting room.
For your safety: No guns, knives or weapons are allowed on premises.
